Advanced Home Care's We’ve Got Heart at Home disease management program is designed to reduce the number of hospital admissions for Congestive Heart Failure (CHF) patients by enhancing treatment compliance.
Because self-care is of utmost importance when treating Congestive Heart Failure, we educate patients about the disease’s symptoms and progression, fluid management through daily weighing, best nutritional options, medication scheduling and early recognition of acute symptoms. We also schedule our in-home visits to ensure maximum support for patients when they first arrive home from the hospital, a strategy that significantly reduces the rate of hospital readmission. And, we provide ongoing support to patients through the subsequent weeks and months to encourage their continued compliance.
Our comprehensive in-home assessment also includes screenings for oxygen deprivation and obstructive sleep apnea. Patients who score high on either of these evaluations are referred for further testing and assistance through our respiratory therapy department.
