Advanced Home Care’s Chronic Obstructive Pulmonary Disease (COPD) Program is designed to improve the quality of life for patients with Chronic Obstructive Pulmonary Disease and reduce hospital readmissions through the implementation and management of an individualized care plan.
We teach our Chronic Obstructive Pulmonary Disease patients about the disease’s symptoms and progression, how to avoid irritants, medication management, best nutritional options, ways to conserve energy, breathing tips and exercises and early recognition of acute symptoms. Our strategic approach to scheduling in-home visits helps ensure maximum support for patients when they first arrive home from the hospital, an approach that significantly reduces the rate of hospital readmission. And, we provide ongoing care to patients to encourage their continued compliance.
Our comprehensive in-home assessment also includes screenings for depression, a disorder commonly found in patients with Chronic Obstructive Pulmonary Disease. Patients who score high on these evaluations receive extra assistance to ensure both their physical and mental well-being.
