Information Required for Skilled Services Referral:

  • Dates services need to begin
  • Home-bound status
  • Orders for treatment including specific orders for labs/wound care/medications
  • Ordering physician
  • Skilled services needed (Nursing, Occupational Therapy, Physical Therapy, Speech Therapy, Medical Social Work, Home Health Aide)

To meet Medicare criteria, skilled services must be intermittent in nature and should generally be expected to end at some point (Chronic care is not covered under Medicare).
For most patients, a willing and able caregiver should be available when the patient is unable to care for himself (Patient must be home-bound).

Definition of Home-Bound Status:

For Medicare and most insurance companies, it is usually required that the patient is home-bound. Home is the best place to provide care.  Although these interpretations can be widely debated, home-bound is generally interpreted as meaning “a significant hardship to leave the home” (lack of transportation does not qualify as home-bound).

  • Diabetic teaching
  • Disease management programs (Congestive Heart Failure, Chronic Obstructive Pulmonary Disease)
  • Infusion services
  • Joint rehab program
  • Medical social work
  • Occupational therapy
  • Physical therapy
  • Speech therapy
  • Skilled nursing
  • Wound care (able to perform assessment and recommendations)
