Patients who have undergone a total hip replacement (THR) or total knee replacement (TKR) can benefit from Advanced Home Care’s Safe Paces program. 

Targeted Clinical Outcomes:

  • Passive knee extension 0-5° and > 95° flexion 2 weeks post op - TKR
  • Full active knee extension and 110° flexion 4 weeks post op - TKR
  • Independent in ambulation to 400 feet or greater - TKR/THR
  • Strength: 4/5 - THR/TKR
  • Dynamic Balance: 4/5 - THR
  • Preparation for outpatient therapy

Program Features:

  • Admit patient within 24 hours of discharge
  • Front loading of therapy frequencies
  • Care Team communication of patient progress by first follow up visit with surgeon
  • Occupational Therapy (OT) to evaluate need for instruction on aids to daily living (ADL) accessories
  • Provision of and instruction on Continuous Passive Motion (CPM) products, as ordered
  • Coumadin monitoring program
  • Proactive pain management protocols


Our rehabilitation therapists begin working with patients within 24 hours of discharge from the hospital and are available seven days a week to help manage pain, reduce swelling and ensure patients regain maximum joint functionality. We also provide any medical equipment patients may need – such as crutches, a cane or a walker – during the initial recovery period and can assist with any other therapies the physician may prescribe.

We also offer prothrombin time/international normalized ratio (PT/INR) monitoring for patients who have been prescribed anticoagulation medications following total joint replacement surgery. Our at-home testing method allows Advanced Home Care’s skilled nurses to check a patient’s blood in minutes, consult with a physician or pharmacist immediately to make any necessary dosage changes and update the patient about the new dosage requirements.
